Void where prohibited. Contest begins 2/25/2010 and ends 3/25/2010. Open to legal U.S. residents, age 18 or older with a valid driver’s license. Employees of City Publications, mail house, American Hole'n One (AHNO), associated sponsors or agencies, and their family members and members of the same household are ineligible. Contestants will have a chance to win the grand prize by entering the original number printed on the mail piece and on file with AHNO. Available prizes, approximate retail value and odds of winning: $5,000 cash (1:40,000). Maximum of one prize awarded per prize level. In the event of error, the number match contest is void and no prizes will be mailed at a future date. All validly claimed prizes will be awarded. Any unclaimed prizes will not be awarded. Sponsor not responsible for late, lost, misdirected or damaged mail. Contest sponsor: City Publications Santa Barbara-Ventura.
Claim Procedures – Requirements for reimbursement of a prize claim:
1. Failure to adhere to any of the terms and conditions outlined herein will result in a claim denial.2. City Publications SB-Ventura must notify AHNO within one (1) business day of any winning redemption.3. In order for the grand prize to be awarded, an eligible contestant must:a. Be the predetermined winning contestant on file with AHNO;b. Have received by mail the original mail piece printed with the predetermined winning number;c. Have entered correctly their name, address, phone number, and the predetermined winning number on sponsors web-site contest after 2/25/2010 and before 11:59 p.m. Pacific Time on 3/25/2010;d. Be the individual addressee printed on the mail piece, and reside at the address to which the winning mail piece was mailed;e. May not have been contacted in any manner other than the direct mail piece.4. Entries that are late, incomplete, unreadable, inaccurate, or otherwise not in compliance with the rules will be disqualified.5. The original number printed on the mail piece must exactly match the predetermined winning number that was provided to AHNO prior to the start of the contest. The original number must not be altered, manipulated, copied, damaged, hand printed, or forged in any manner.6. The name and address on the contestant’s driver’s license must match exactly with the name and address both (a) printed on the mail piece, and (b) provided to AHNO prior to the start of the contest. Winner must provide proof of residence at the address on mailer. No one may attempt this contest on behalf of another individual, or using a mail piece addressed to another individual.7. AHNO reserves the right to require confirmation of the actual number of mail pieces distributed for this contest from the fulfillment company or mail house (U.S. postal service form 3602 postal certifications).8. All taxes are the responsibility of winner and winner must supply a valid social security card and complete a W-9 form before the grand prize will be awarded. An affidavit of eligibility and liability/publicity release form must be signed by the winner and returned to AHNO within 14 days of notification prior to prize being awarded.9. In the event of a claim, a polygraph test (at the expense of the sponsor) given by a professional polygraphist may be required of anyone involved in this promotion to further verify any claim. A polygraph test that reveals any fraudulent activity or non-compliance with any of the terms and conditions outlined herein may result in a claim denial.10. Neither AHNO nor CityPublications SB-Ventura is liable for the non-delivery or delayed delivery of any mail piece by the U.S. Postal Service, mail service, and/or any promotional agencies involved in this contest.
